119.03 (A) Public liability insurance required by division (A)(13) of section 1514.02 of the Revised Code is effective from the date of issuance of the permit until the release of the final portion of the performance bond. (B) A rider requiring that the insurer notify the chief of termination of the policy or failure to renew is necessary for any public liability insurance policy. (C) The permittee's proof of insurance coverage may be submitted on a form provided by the chief or on a form provided by the insurance provider that includes, at a minumim: (1) The name of the permittee covered by the policy, or if the insured is not the permittee, the name(s) of the operator(s) or operation(s) covered by the policy; (2) The policy number; (3) The policy period; (4) The amount of coverage; and (5) The name, address, and telephone number of the insurance provider. (D) At the request of the chief, the permittee is to provide proof of insurance.
