Section 4540.20  Applicability

This Part

applies

to any

individual or group

policy of accident and health insurance

, health maintenance organization

health care plan, or voluntary health services plan when the insurance or plan

is

a network plan issued, delivered, or renewed in Illinois

.

It

does

not apply to any individual or group policy of dental or vision insurance or a

limited health service organization

. (Section 3 of the Act) This Part also

does not apply to the medical assistance program under the Illinois Public Aid

Code.
