Section 309.110  Contents of

Public Notice of Application

The contents of the public

notice of applications for NPDES Permits must include at least the following:

a)         Name, address, and telephone number of the Agency;

b)         Name and address of the applicant;

c)         Brief description of the applicant's activities or operations that

result in the discharge described in the NPDES application (e.g., municipal

waste treatment plant, steel manufacturing, drainage from mine activities);

d)         Name, if any, of the waterway to which the discharge is made

and a short description of the discharge's location indicating whether it is a

new or an existing discharge;

e)         A statement of the tentative determination to issue or deny an

NPDES Permit for the discharge described in the application;

f)         A brief description of the procedures for formulating final

determinations, including the procedures for submitting comments and the expiration

date of the comment period; and

g)         Address and telephone number of Agency premises at which

interested persons may obtain further information, request a copy of the fact

sheet, and inspect and copy NPDES forms and related documents.
