DATE: MARCH 26, 1996

SUBJECT: BULLETIN 91-6 (REVISED)

FORM FOR PAYMENTS TO VOLUNTEER FIRE DEPARTMENTS

As required by H.B. 270, 1991 legislative session, we have prepared a uniform form to be used by fire departments for minimum payments for initial fire responses effective July 1, 1991. As you may know, the new statute requires any insurance company selling any type policy which includes fire insurance to pay a minimum of One Hundred dollars (100) to volunteer fire departments responding to fires in protection classes 9 and 10.

It is hoped that companies which permit payments to other departments and fires in other classes will utilize this same form to process payments. Please feel free to contact the State Fire Coordinator, or my staff, if you have any questions.

Commissioner of Insurance
