The following safety violations ware f-und o- line om

in an inspection made by Lt. I. eS

Retruary 1947.

1. Bldg. 1 = The Towmotor fork lift #399 has an

accumilation of grease and oil om the transnission. The radiatar

cap was missing and horn was inoperative.

26 No fire extinguisher on Mercury Trector £51 and the

horn was inoperative.

There was corosion on the l'ft of towmoter without

a mumber. Motor was greagy. O11] and grease o. transmission. The

horn was inoperative. No radietor cap.

he The horn on towmotor #58 was inoperative.

In accordence with ordnance safety bulletin 100, for’ lifts

should be kept free of grease ani oil. And regular inspection

should be set up as the horns on the above on the above mentioned

fork lifts were inoperative approximately three weeks ago.

es 5- A large number of cigarette butts and burnt

matches were found on the floor near the fire doars in the south

‘bay of ll-l.

6. A guard should be installed ower the pulleys and

V belt which is located in the pit.

8 The 2 x 12 planks shold be rempved fre the rafters

over south ‘gooling cylinder.

9. The ofl] and grease should be deansd off the floor eni

drums in te oil storage section Bldg. 2.

10. The Ro-ball sifters and Karon sotles located in the

_morth end of Bldg. ? should be cleaned and coated with a rust and

corosio: preventative.

ll, The compressor units and the floor of Bldg 3 should be

cleaned, There <a a large acoumlation of oil.

12. Egg shells, rags and paper have been stuffed in the center

boiler in the boiler house. Rags were foun’ behind the box in the SH.

5. In regard to Ites four, "Feasibility of Bagging Temperatures

Lower than 190 vegrees, and if Feasible, Determine the Cost of Additional

mode in atteapting to reduce the bagging tenperatures of fertiliser.

However, eontimmal effort ie necessary and the importance of reducing

the bagging teaperature further eannct be emphasised tro freatly at

efforts will undoubtedly be réwarded by continual production in the future.

6. In commeetion with Item one, ef i art two of the above telersran

procedure be subaitted tis office, as soon as possible, stating the

action taken er the proposed action necessary to effect this reduction

effect the desired results be inaluded.in the proeedure requested in

paragraph 6 above,

1851. Inspection of Ammonium Nitrate High Pans. In view of suspected

hazardous conditions prevailing at the nigh pans of the ammonium nitrate pro-

cessing unit, the following inspections snould be made;

a.

Remove and inspect air agitators (lances) to determine if

ammonium nitrate has been deposited; also to deteruine if

the desposits contain caroonaceous matter or other impurities,

Inspect air supply pipes and agitators to cetermine if source

of air pressure is likely to introduce oil into the lines

and thence into the liquor in the high-pans.

Investigate process to determine if any of the coatea nitrate

can be reintroduced into the high pans.

Operation “at the ammonium nitrate high pans shoula not be

accomplished with a saturated steam pressure greater than

100 pounds,

The following corrective measures are recarmended for unsatis-

factory conditions: 3

(1) Air agitators (lances) to be provided with a drain in

the bottom near the dead end of each pipe to prevent

accumulation of nitrates,

(2) Where plunger type compressors are used and the possi-

bility of oil being introduced in the system prevails

~ piping should be changed to provide a piping layout

that will prevent accumulation of oil between the filters

and the high pans,

1. Tt is the understanding of the undersigned that the Operator

Contractor of the Nitrate Plant, contemplates the operation of a chemical

laboratory in Building N-2.2 This would necessitate the presence of

Ammonium Nitrate, various including volatile liquids, gas and

electrical heating equipment.

2. According to Ordnance ety Manual OO Form No. 7224 dated

3 May 1945, Ammonium Nitrate is Classified as an explosive.

3. Building N-2, in addition ‘to containing the chemical laboratory,

will also contain offices and will house most of the Contractor's office

personnel, and would therefore be considered an inhabited building. The

hasard involved in the operation of this laboratory would be applicable

to all the office personnel in addition to the laboratory personnel.

4. It is a know fact that the sensitivity of Ammonium Nitrate

increases if the nitrate is contaminated with certain chemicals and if

the nitrate is in certain physical states. It is reasonable to assume

that the possibility of chemical contamination and the presence of

Ammonium Nitrate in various physical states would be greatest in a

chemical laboratory. 3

5- The presence of gas and the possible presence of fumes from

volatile substances not only constitute a fire hazard, but may be con-

sidered a health hazard and may even effect the efficiency of the office

personnel that may be subjected to it.

6. In view of the above, it is suggested that the presence of the

above described laboratory, in an inhabited office building, be investi-

gated from a safety viewpoint.

lations which are shipping materials and equipment for the Ammonium

Nitrate Program.

2. There is a likelihood that some of the equipment and materials

shipped to your installation from other installations under the super-

vision of tnis office, for use in the Ammonium Nitrate Program, will be

contaminated with cnemicals or explosives You are directed to consult

with the operating contractor and the construction contractor per forming

the rehabilitation and construction necessary at your installation for

the ammonium Nitrate Program to determine that they are fully advised

and cognizant of the above condition and that proper precautions are

taken in the method of handling such equipment and material and that such

equipment and material will not be used in a manner that will constitute

a hazard in future operations, either at your installation or any other

installation engaged in the Ammonium Nitrate Program. Particular empha-

sis should be given to the absolute necessity that equipment, pipe, valves

and fittings, whicn are to be used in the preparation and handling of

aqueous solution of ammonium nitrate, be free of nitro bodies o= other

(FERTILIZ:R GRADE)

The following changes are made in FDAP Specification for

Ammonium Nitrate (Fertilizer Grade) dated 8 July 1946:

Pare 2. Delete paragraph F-2 and substitute the following: -

"F-2. Inspection. Inspection shall be conducted as

follows: Samples shall be taken as bags are being filled. An eight

ounce sample shall be secured from approximately every one hundredth

bag using stainless steel sampling scoops. Each sample shall be identi-

fied with the material to which it pertains, as completely as possible.

All portions of each lot shall be required to meet the requirements out-

lined in Sections D & E above. 4

Dear Sir:

Mr. Roy Calkins, Works Manager of the nitrate plant at your installation, has

just called me by long distance telephone and read a letter in which you ob-

jected to the method used in the contractor's laboretory in the determination

of water insoluble material present in grained ammonium nitrate fertilizer.

I refer you to the attached photoetatic copy of a letter I received fram

Mr. Cottrell of the Pield Director's office in which he stetes he would have

no objection to the laboratory techriques I had previously asxec permissicn

to use in Emergency Export's laboratories.

The procedures I described in my letter to Mr. Cottrell were the current ones

Being used in our laboratories, nemely: the determinetion of water ins>lubdles,

the use of carbon tetrachloride in place of ether and the use of a different

normality of sodium hydroxide than used in F.D.A.P. specifications.

When Mr. Rapp was here in December we talked further on the methods being

used in our laboratories, and he was very well satisfied with our methods and

is seriously considering adopting them into the present specifications. “e

In regerd to the.use of carbon tetrachloride, information has beer submitted

to P.D.A.P. to prove its superiority to ether for use as the solvent in

~ extracting the "ether solubdles".

niques" employed in our laboratories I wish that they might be directed to

this office as all the methods employed in our laboratories originate in this

office.

excess temperature. 7DA. Specification for Ammonium Nitrate

(Fertilizer Gr-de) dated & July 1°46 paragraph G-lb states, in

cart, that “asphalt used shall have a melting point of approximately

200°F., if possible, ut in no case shall this be less than 175°F.

It is requested that your office stipulate a maximun temperature

at w ich fertilizer may be loaded in order to eliminate the

above mentioned danger.

his station should be prepared on the attached Vendor's Shipping Document

as used by The Military Chemical Works, Ino.

2. For each oarload of fertilizer grade ammonium nitrate shipped,

seventeen (17) copies of the dooumentM#will be required. These copies

will be distributed by The Military Chemical Works, Inc. as follows:

Three copies mailed to consignee designated by Commodity

Credit Corporation. On two of these copies, the following

notation should be stamped in the left-hand margin:

SUBJECT: Safety Valves and Vents on Ammonium Nitrate Tank Cars

1. Recently, several complaints have been received from solution and

graining plants that safety valves and vents on ammonium nitrate cars are

in unserviceable conditicu due to spikes being placed in safety valves and

yupture discs being reinforced or replaced with material other than the

lead disc prescribed by ICC regulation, thereby rendering the safety device

inoperative.

.

2. These conditions create a definite hazard both in transit and in

the unloading process as well as contribute to the rapid deterioration of

the tank cars by allowing excess pressure to build up while the car is being

unloaded, causing ummecessary expease and loss of car days for repairs and

further reducing the already snort tank cur supply.

3. It is primarily the responsibility of the.shipper to inspect and

repair all safety valves dnd vents on tank cars prior to loading (see Sec-

tion 31, Freight Tariff #4 ICC #4 Transportation of Explosives and Otner Dan-

gerous Articles by Freight). However, it is impossible to keep cars ina

safe condition if the unloading plant does not assume the responsibility of

inspection and seeing that safety appliances are operating vefore attempting

to unload the cars.

4. It is, therefore, requested that each tank car be inspected prior to

loading and unloading to see that safety appliances are in proper operating

condition and a supply of rupture discs be kept on hand for replacement. When

a safety valve is found to be inoperative, it is to be reported to this office

for disposition. This is usually a shop job and should not occur frequently.

Furnish a report in each instance when a car loaded or empty is received with

safety appliances not in operating condition, showing car number and plant from

which it was shipped, in order that corrective action may be initiated by this

office to avoid further violations of safety regulations.

‘ | 1. Materials ourchased by contractors for incorporation into a final

: product which is subject to Government acceptance may, or may not, be sub-

5 ject to inspection by Government inspectors. Although Government speci fi-

cations are usually applicable to such materials, inspection py Government

inspectors is made at the request of the contractor. Usually such a request

is made when the vendor is unable to furnish a certification as to the on-

pliance of the material with the specification requirements, or when the

contractor is unwilling to accept such certification. Im such cases, for-

mal acceptance for the Government, as expressed by issuance of an OO Form

1010, is not accomplished when the inspector completes his examination; in-

stead, a st:tement of the inspector's findings may be sent to the contractor

for approrriate action. This does not interfere, in any respect, with the

Contracting Officer's (or C.O.R's) right to examine any constituent or com-

ponent of the final product for compliance with applicable specifications.

2. In order to furnish a uniform basis for procurement of clay, Wax B,

and PRP, specifications were prepared and distributed in July 1947. To date,

these have not been chanred. Some difficulties have been encountered in pro-

curing clay, however, which will necessitate some changes in that specifica-

tion as soon as arreement can be reached and differences resolved. The am-

monium nitrate specification dated 1 October 1947 proviced, under Par. C-ld,

that requirements beyond those included in the clay specification could be

stipulated by contractors in order to procure a clay “suitable” for their

purposes. The Emergency Export Corporation may have taken advantage of this

provision to insert additional requirements based on their operating experi-

ence. This office would have no kmowledge of such an action or of the added

requirements. All such detailed data, together with correspondence, and

other supporting data, is available through purchase orders furnished your

office by Emercency Export Corvoration's central procurement group.

! I am quite sure that you have read in the newspapers that the Nebraska

Ordnance Plant at Fremont, the Iown Ordnance Plant at Burlington, and the

Cornhusker Ordnance Plant at Grand Island will be operated in the new Govern-

ment Pertiliser Programs and it is alse possible that you my have learned

that Spencer Chemice] Company will operate these three facilities. It dees

not ‘seem provable that operations will actually start before the midle ef

‘September, but the preliminary contracts have been sisned and the definitive

contracts are being negotiated at this time. These eontracts should be com

_- pleted within the next few days, and Spencer Chemical Company will agein

operate the Cornhusker Ordnance Plant but under another contrect,- se it »ill

be necessary fer us to close out the accounts of one contract end reopen thea:

Grand Island, Nebraska

1. An inspector, spot checking the quality of material being produced

and accepted under a GOCO cortract, should not confine his activities to

laboratory examination of samples. The scope of his inspections must be ex-

tended so that he can assure himself and his Commanding Officer that all of

the product accepted complies with the requirements of the contract and the

specificatiom In order to do this he must become familiar with all plant

operations that bear m the product, its processing, and its quality. It

is essential that he become familiar with, and understand, the controlling

factors and tasic control points in the process. With this knowledge he is

in a position to regulate his laboratory examination and point his anal yti-

cal testing to variations in operating procedures. It is emphasized that

. plant inspections and reviews of operating procedures and controls are not

ends in themselves and that interference with operations or production can-«

not be permitted,

2- Inclosed is a partial list of points that should be checked pericdi-

cally by the Government inspector. Variations in plant layout or desig will

automatically eliminate some points and introduce others. Each inspeotor will

probably have several items to add to the list. Obviously, all points cannot

be checked daily and some others not every week; however, all points should

be kept under scrutiny. It is requested that the attached list be made a

part of the Government inspector's routine spot check procedure.

SUBJECT: Packing Temperature for Ammonium Nitrate Fertilizer

TO Commanding Officer

1. This letter rescinds all previous instructions concerning packing

stemperatures of ammonium nitrate fertilizer and the reporting of observed

temperatures af bagged material.

2.: It is now, and will continue to be, a requirement that ammonium

nitrate fertilizer shall not be packed at a temperature exceeding 190° F

. (88° C.).

3. Considerable evidence has been accumulated to demonstrate conclu-.

sively that at temperatures above 210° F deterioration and degradation of

multi-wall paper bags may be expected. Furthermore, these conditions will

be encountered at lower temperatures when the bags have been exposed to,

or have come in contact with, any of a variety of extraneous materials (oil,

water, ammonium nitrate solution, etc.).

4. Studies made on bag temperatures show that the 190° F bagging tem-

perature limitation is being observed in general. All plants, however, have

experienced same instances where the 190 F limitation has been exceeded.

Such a condition is to be expected from a probability point of view. For

this reason, a l percentage of bags packed with fertilizer at tempera-

tures above 190 F will be permitted. Because of the deleterious effect

on bags, fertilizer at temperatures of 200° F and above will not be permitted

to remain in shipping bags.

5. The inspection procedure to be followed in checking this character-

istic is outlined herewith: a

a. It is assumed that all cars have capacities of between 700 and

1200 bags. On this basis, the temperature of the fertilizer in 115 bags

(about every eighth bag) from each carload will be obtained and recorded.

Of the bags inspected for each carload no mora than eight (8) will be per-

mitted with temperatures between 191° and 199°’ F. If more than eight (8)

bags with temperatures between 191° snd 199° F (or any beg with a tempera-

ture over 200° F) are found, it will be necessary to remove the bags from

the car. If the limit of defective bags is reached (or if a bag over 200°F

is found) before the car is finished, the loading of the car will be stopped

at that point and corredtive action taken. If the car is unloaded because

procedure is not in accord with Amendnent 2, ye F-2 of FDAP

Specification for Ammonium Hitrate (Fertiliser Grade).

S. It should also be pointed out that the continuous change

in methods of inspection and ecoeptance of Ammonium Mitrate (Ferti-

liser Grade) has worked an extrend hardship on this facility. It

has necessitated the submission and later the withdrawal of three

le It in requested that a sample be taken of the emmoniunm nitrate

dust in the central bagging house. The sample should consist of approx-

imately twe (2) quarts and should be thoroughly mixed. Analys s should

be mde fer moisture, ether soluble, «ater insoluble, and nitroven content

by the specifieation procedures. The hygrescopicity of the dust shouad be

measured at 90% relative humidity and 30°C. Ome (1) quart of this sample

should be sent to Picatinny Arsenal. J

fellewing tests should be conducted on representative bags from each sup-

plier. The moisture content of the ba;s will be determined prior to test,

and the necess-ry number of bays with less then 7% will be selected. A

mumber ef das of each type will be conditioned to give moisture cont-nts

. of approximately 8, 10, 12, and 15%. Ome (1) series of bags will be dusted

: lightly with ammon‘um nitr te fertiliser dust, a control sesies will be used

without nitrate dust. Tests will be conducted on dusted and wdusted empty

bags as well as those filled with fertiliser.

l. An inspection of all shipments of paper bags held in sterage st

this Arsenal, has been nade, and all recent shipments meet tne specifica-

tions as regards construction. These ere currently being used on the

line.

2. However, there are several carloads of bags manufactured by The

International Paper Company, New York City, trade-name "BAGBAE", received

several months ago, which do not meet the specifications due to the fact

that they have one or two extra layers of Kraft paper ever the outside

asphalt laminated layer.

3. The contractor: has been instructed to purchase only bags which

meet the specifications. The ba;s mentioned in Para. 2 wili be withheld

from use on tne line pending a decision from your office.

Dear Joel:

In acoordence with your request mde in telephone conversation wit

the undersigned this date, inclosed is a record of the temperature violations

by the contractor in the manufacture of ammonium nitrate fertilizer, which

were discovered by my inspectors in their one or two checks per shift. If

my inspectors find this unreasomble number of high temperatures in their

infrequent checks, I can only assume that high temperatures prevail during

the other six or seven hours of each shift,

It is again recommended for the reasons stated above and in our.

let indorsement dated 3 July 1947, ORDSB-E 464.6/234, (ORDLY-I 471 .86/17560

Ravenna), that my per sonnel ceiling be increased by five (5) graded employee

in order that I will have an inspeotor on duty at the Ammonium Nitrate Plant

100% of the time to report temperature and safety violations, and other viol-

ations of instructions; such as, incorrect loading of cars, and improper

count of the number of bags in the oars, as well as underweight and over-

weight bags. I have outlined a standard operations procedure for these

proposed inspectors and would have them make written reports to me which I .

will pass on to the Plant Manager with instructions to correct deficiencies.

This is the only manner, in my opinion, in which I can control the operations

and protect myself from oritioism from your office and higher authority,
