Part 1 (Positions) and Part VIT at line 15 (Investments and Trusts):

FOR THE YEAR ENDING 12-31-14, 1 REPORTED AS FOLLOWS:

Some time ago 1 was numed Sole Trustee and Attorney in Fact for a "Living Trust" created by a Trustor who was a relative. I had no role or authority, pending the

Trustor’s death or incompetency.

On June 19, 2014, the Trustor became incompetent and | became the Sole Trustee and Attormey in Fact, with complete authority over the assests of the Trust,

1 took no action and made no decisions for eleven days. On July 1, 2014, by written instrument, I delegated my entire authority to another as Trustee and Attonrey

in Fact.

Thereafter and throughout the latter part of 2014, this succeding Trustee liquidated Trust assets, and moved the value to instruments outside of the trust. This did

not rerquire my approval , control or acknowldegment,

(Evidently this is a commonly-recognized way to qualify the incompetent Trustor for Medicaid, while

maximizing the assets remaining for the marital community.)

On December 31, 2014 the sole remaining asset of the Trust was a bunk account containing $1, 877.26,

FOR THE YEAR ENDING 12-31-15

The Trustor passed away on 5-4-15

The bank account referenced above was closed by my successor trustee on 5-26-15

For the year ending 2016, 1 expect to check the box "NONE" in Part 1
