I have disclosed the reportable assets that were held by Trust #2. Partial in-kind distributions of those assets were made to me and the other two beneficiaries in

2014. Final distributions of the assets were made to me and the other beneficiaries in 2015.

With respect to Trust #3, the assets held by the trust at the time of the relative’s death were liquidated in 2014 within weeks of my relative’s death and invested

in a money market account, und 1 received a cash distribution in 2015. 1 was not aware, prior to the liquidation, of the identity of the securities or investments

held by the trust and subsequently liquidated and invested in a money market account. | have identified the money market assets held by Trust #3 at the end of the

2014 reporting period.
