In Section VII, State Street made mandatory investment lineup changes in 1st quarter 2016. SSGA Russ Lgepgr Ldx assets were moved into SS S&P 500 Index

which is an existing fund in my wife's account. The value of J is correct. SS Mid Cap Index and SS Russ SM Cap 1dx assets

were moved into SS Rus SM CP C IDX 1. These assets were incorrectly labeled in the prior Financial Disclosure as SSGA Russ SM Cup Idx. All State Stree funds

were incorrectly labeled as SSGA on the prior report, and should be just SS which stands for State Street.
