2. Fidelity Investment Asset Manager - Upon seeking additional information from Fidelity investments, on 12/2002017, information provided reflects that they
had closed the account in September 7, 2011 and available funds were sent to state governmnet as "unclaimed funds." No prior notice had been received.
3. The funds now managed by Wells Fargo, Advantage Funds were previously managed by Strong Funds. The transfer of brokerage results from a company
merge. The Opportunity Funds are invested primarily in mid-size companies, blending the type of investment (Ex: Value & Growth),

AXA/Equitable Life Insurance: The fund pursues its investments objectives by investing in other mutual funds (over 6,000) managed by AXA Equitable
Funds. Approximately 70% of its assests are invested in equity assets class and 30% in the fixed incom assets cluss through investments in underlying funds.
There is not one specific identificable mvestment. The top 3 holdings for example were: ATM Large Cap Managed Volatility K (16.27%); EQ Intermediate
Government Bond K (12.49%); EQ/Core Bond Index K (8.86%), etc.

In late 2001 or carly 2002, I found out that my father had opened this savings account under my name. Thus, it was reported in the AO-10 financial reports of
2002-2004. (Upon my father's death on January 20, 2003, suid funds were transferred under the estate and/or my mother's name).
7. CD Account, Banco Popular de P.R. | appear as co-signer/co-owner. Funds belong to my mother.
8. Real Estate Property No. | - Residential Property: (12.5% Interest) Puerto Rico. Appraisal date: August 6, 2003. Since 2003, property is part of the family
estate.
9. Real Estate Property No. 2 - 29 acres of land: (12.5% Interest) Puerto Rico. Appraisal date: August 6, 2003. Since 2003, property is part of the family estate.
10. Centro Agricola (Agricultural Center). My interest of participation was that of # 12.5%. Had no participation in daily operation of the business or
managerial decisions. No income or benefis were received. Bussiness closed/ceased operations. Property was part of the family estate and only the “inventory”
was owed and disposed of.
11. Oriental Bank Trust - CD IRA account opened 5/19/05 includes amounts rolled over from item #1 | within the 2005 Report. The new reported balance
includes a transfer of Item #4 above as of 5/27/2009
