Part VII, lines 3, 4 - UIL merged mto Avangrid in December 2015. Existing shareholders of UIL acquired Avangrid stock plus $10.50/shr. In my case, that
$10.50 is taxable as long-term capital gain which is how I've reported it here. Spinoff is the closest description for type of acquistion that the drop down menu
allowed, but technically it was a tax-free exchange with taxable boot,
Part VII, lines 25-27 - 1 reported this as a merger, but both the old funds continued in existence for some investors. My holdings were in a 401k at a former
employer, and the custodian of that plan exchanged them in 2015 for an equivalent value of Vanguard S&P 500 Index Fund shares. My year-end statement
showed the change but not a specific date for the exchange.
