Part VII. Investments and Trusts

Re - Prudential / Herzfeld & Rubin, P.C. Employees' 401(k) Plan. Lines 5, 6, 7, 8, and 9. Nothing has been contributed by Herzfeld & Rubin, P.C. (a former

employer) since I left the firm's employ in 1999. I do not make contributions. 2019 is the first year I made any withdrawals. I indicated positive changes in value

as int/div. I withdrew funds each month beginning in May 2019 through and including December 2019. The form requires me to select one date so I picked

the date of the first withdrawal. However the withdrawal dates were as follows: May 15, 2019; June 17, 2009; July 15, 2019; August 15, 2019; September 16,

2019; October 15, 2019; November 15, 2019 and 12/16/2019. Prudential allocated the withdrawal against all of the holdings in the same percentage basis as each

holding represents to the total dollar amount of all the investments in my plan, and in the category amounts I listed in Part VII D as partial redemptions. Please

note that in D(3) Value Code 2, I listed the value code for the total withdrawn over the entire 8 months for each investment and not the monthly withdrawal.

Part VII. Investments and Trusts

Item 4 - A dividend is earned each year on the anniversary of this whole-life policy which automatically results in the acquisition of additional insurance coverage.

Since the anniversary date is in June each year, I have included the dividend amount for 2019, (the dividend amount would fall within category "C" but the amount

of additional insurance that the dividend purchased falls within category "D", so I used category ("D"). For "gross value" at end of the reporting period, I have

provided the cash surrender value information which falls within category "K".
