However, | continued to perform duties as a recall Magistrate Judge. | ended my carreer on August 31, 2015, after serving as a Part Time Magistrate Judge

handling Social Security cases for the MDFL. 1 had no office or chambers, My assigned duty station was my home. | have no other address to list on the form,

In Part VIL, lines | and 2, | listed the mterests | have in the Indiana College Fund (a 529 Plan). Throughout 2015, | made equal monthly contributions to two mutual

funds in each of ten separate accounts in the Indiana College Fund, one account each for my four children and six grandchildren, The contributions were $50 per

child and grand child per mutual fund per month, or a total of $100 per month for each child and grandchild (8 payments, from Jamuary through August), The total

of all of these contributions in 2015 through September 1 was $8000.

In Part VIL, lines 3 and 4, | listed the mterests | have in the lowa College Fund (a 529 Plan). Throughout 2015, | made no contributions to the fund, but |

maintained interests in ten separate accounts invested in two separate mutual funds in the lowa College Fund, one account each for my four children and six

grandchildren.
