For part VII of the report, relating to Investments and Trusts, there were two purchases in 2014 in my brokerage cash account that were mistakenly omitted from

my report for calendar year 2014,

QCOR Common Stock is listed on line 4, The stock was purchased on July 23, 2014, and at the end of the 2014 reporting period, the value code for QCOR was

"J" and the reporting method was "T." QCOR was sold in 2015 as indicated on line 5.

FB Common Stock is listed on line 9. This stock was purchased on March 24, 2014, and | continue to hold it. At the end of the 2014 reporting period, the value

code for FB was "J* and the reporting method was "T.*

Additionally, the 2014 report indicates that part of my holdings in CLVS Common Stock were sold in August 27, 2014; the sale on that date, however, was of all

CLVS Common Stock. For that

reason, CLVS Common Stock does not appear in this report.
