Health Retirement Plan. The filing instructions issued January 3, 2006, at pages 43-44 now state that "[a]ssew held in tax-deferred retirement or pension accounts,

including 401(k), 403(b) . . . qualify as ‘common trust funds," and "[t]herefore, the filer is not required to provide any details about the individual assets held by

those plans, but is only required to list the name of the specific plan in Column A." Accordingly, lines 5-11 and lines 13 and 14 of the Report for the year 2004

have been deleted as they no longer meet the reporting requirements.
