amended 2016 report filed March 31, 2018. In that report, box 17(A) should read "CA Scholarshare Passive Age Based Portfolio, # 13-14 - 2940", and box I8(A)

should read "CA Scholarshare Passive Age Based Portfolio, # 11-12 - 2939". The information listed erroncously refers to the 2018 asset classes.

The reported Wells Fargo and Chase Checking and Savings Accounts are of reporting individual only and exclude accounts of ex-wife pursuant to marital

dissolution. Date of separation is January 31, 2016, and effective date of divorce is March 19, 2018.
