FN *. Re Part 11 ("Agreements"), and Part VII ("Investments and Trusts") lines 3 through 5: The Wildman Harold Retirement Savings Plan (later known as the

Edwards Wildman Retirement Savings Plan) managed by Mass Mutual Financial Group was reorganized and merged into the Locke Lord LLP Retirement

Savings Plan, managed by Milliman Benefits, in 2017. With the merger, my retirement funds that had been invested by the former plan in the Invesco Stable

Value Fund were rolled into the Vanguard Federal Money Market Fund. In addition, my remaining funds formerly invested in the Mass Mutual MML Income and

Growth Portfolio Fund were rolled into the Vanguard Target 2020 Retirement Fund. 1 was not involved and entirely passive in this reorganization and merger of

the former plan which resulted in this listing of these new assets (Vanguard Fed. MM and Vanguard Target Retirement 2020) as described. | realized no taxable

gain by this action.

FN ** Re Part VII (“Investments and Trusts") lines 22 through 24 (entries for “Estate”:

This is the estate of a deceased family member, The estate is in the process of closing and is attempring to locate, identify and recover any assets to which it may

have a claim, The estate has been informed that the deposit accounts(s) to which it may have 4 claim (the "Fifth Third Bank deposit account(s)") may have been

turned over to the State of Hlinois by the bunk. The estate is attempting to identify and recover any recoverable funds attributable to this'these account(s). The

value reported on line 24 represents the estate's best estimate of the possible value of the account(s) based on scattered and imcomplete mformation and does not

represent their actual value, if any. The estate is not aware of any related interest earned in 2017.

The estate also is attempting to ascertain the deceased's possible holdings, if any, in common stock of the former NYNEX Corp, US West Corp, Bell South Corp.

and SBC Corp. At this time, the estate is not aware whether in 2017 or thereafter it held/holds any interest in shares of such stock or interest in other stock other

than the common stock identified on line 23 of Part VIL.
