## Text

(a) The department will issue an applicant, whose application
 has been approved, a license containing the licensee's name, license
 number, and expiration date.
(b) An intern in audiology license issued by the department
 remains the property of the department.
(c) The department will issue a duplicate license upon
 written request using a department-approved form or in a manner prescribed
 by the department and payment of the duplicate/replacement fee under §111.160.
