ADDENDUM TO DEFENDANT'S WAIVER OF
CONSTITUTIONAL RIGHTS FORM —- TERMS OF
PROBATION —- (DUI FIRST OFFENSE)

DEFENDANT having been convicted for a violation of
VEHICLE CODE SECTION 23152(a)(b) and legal
cause being shown why judgement should not be
pronounced. IT IS ORDERED that the imposition of
sentence be suspended and that DEFENDANT be
placed on probation from this date on the following
terms and conditions:

2. [(/s/ MT] Pay a fine of $500 Plus a penalty
assessment of $850.00 for a total of
