Part VII, Line 6: Name change. Alive replaced Health Services Credit Union

Part VII, Line 8: (X) The CD was inherited,

Part VII, Lines 12 (the header) through 16 are widely held, publically traded, widely diversified and arranged so that I neither exercise control nor have any ability

to exercise conrol over the financial interests held by the funds. My mvestment advisor states that each of the accounts are invested in "moderate investment

portfolios” which consists of dozens if not hundreds of ETFs or funds.

The holdings are frequently adjusted in percentages of each asset so there would be hundreds of individual transactions each year, over which | have no control.

The holdings

are the same for anyone else investing in the "moderate investment portfolio” category, and not unique to me.

Part VIL, Lines 13 through 15 are the same accounts reported last year, but the investment advisor listed in the header (line 12) chose different management

companies than the year before. The investment in each account remained the same.
