The

Trust was created

during

lifetime. At her death in 2004 significant lump sum payments were made to various charitable

and family member beneficiaries. The Trust is managed by PNC Wealth Managment (A member of teh PNC Financial Services Group). 1 have no knowledge of

the holdings of the trus or the amount in the trust. | reieve quarterly income payments equal to 6.5% of the trust's annual income from the trust. On my death my

interest reverts to the remaining beneficiaries. | have no power of appomtment. Otherwise | have no mvolvement with the trust or its managment.

For IRA #2 (lined) additional shares were purchased thru a matching payroll deduction and by indivival payroll deductions, All purchases were perportional and

did not change the value code.

For IRA #3 (line 12) no additional shares were purchased in 2011.
