In Part VII, on lines 4-6 and 8-10, | listed three Fidelity 401K mvestments as "Open" and three as "Closed," in the Transactions section. These transactions all

involve a change in 401k allocation for those holdings in my 401k, which are listed in my 401k statement as an "exchange." | have no recollection of specifically

making any changes to my asset allocation. Based on my reveiw of the Fidelity account, 1 believe that those exchanges occurred because the plan no longer

includes the assets that | had previously chosen, and thus the funds invested in those “closed” funds were automatically rolled over into the equivalent investment

holdings then available under the plan. None of the three "closed" funds are listed as currently available investment options for this 401k. The value listed is the

value of my investment in those assets on the date of the exchange.

In Part VII, on lines 15 & 16, the Transactions listed reflect the transfer of money from a traditional IRA into a Roth IRA. 1 selected "distributed (part)" and

"spinoft™ for the nature of those trunsactions because those were the closest descriptive terms that 1 found among the available options.
