transfered (rollover) all funds in this managed retirement account into the Thrift Savings Plan.

3. The Wells Fargo HSA account referenced in Part VII is a health savings account | maintained while emplyed with Miller & Martin. | have not made any

deposits into such account since my departure from Miller & Martin on June S, 2015, in connection with my appointment as a Bankruptcy Judge on June 5, 2015
