In October of 1996, 1 became a member of an investment club known as First Investors of Paducah, which is organized as a partnership, There are 15 partners

divided into 7 teams, Each team invests their portion, Tonly have control over the investment of my team. Tdo have a 1/15 interest in all investments.

(No, 3-7) IRA- UBS. The total income from dividends and interest for the account was $15,000 or less,

(No. 8-10) IRA - UBS. This is my spouse's account. The total income from dividends and interest in the account was $15,000 or less.

(No. 13-23) IRA - Regions Bank. The total income from dividends and interest for the account was $15,000 or Jess.

I have an TRA with Regions Bank and UBS. Other than making an allocation choice between bonds, stocks, money markets, etc., | excercise no control over the

IRA account. 1am not consulted nor do 1 give advice on the individual investments.

(No. 24-93) First Investors of Paducah is an investment club organized as a partnership. There are 15 partners divided into 7 teams. Each team makes an

independent investment of their portion. | exercise no control over the investments of the other 6 teams, | do have a 1/15 interest in all of the investments. Items.

24 thru 93 represent the stock investments of the club. Line 24 reflects the estimate of my 1/15 interest in the entire assets of the partnership, My portion of the

share of IBM (26), Simon PC (31), Exon (42), and Cresud (74), have a value of $1,000 to $2,500. My portion of the shares of CSI (41) and Apple (52) have a

value of $2,500 to $5,000 in cach. My portion of the remainder of all stocks held in First Investors, No. 24-93 all have a value of $1,000 or less. My portion of

the dividend income for ull shares held by First Investors No, 24-93 is $1,000 or less in each.
